- Cignal subscriber should completely fill-out the UCPB Auto Collect enrollment form.
- Name of Depositor: UCPB Account holder’s name
- Account Number : UCPB account number
- Maintaining Branch : UCPB branch of account
- Institution Name : Cignal TV Inc
- Institution Code: leave it blank
- Subscriber Name : Cignal TV Account Holder’s Name
- Subscribers Number : Cignal TV Account Number
- Cignal subscriber should submit original and completely filled out Auto collect form to:
Cignal TV Head Office – Customer Experience Center
c/o CIGNAL TV – Credit Team

- The Auto Collect shall take effect only upon approval of the Auto Collect enrollment, and shall apply to the CIGNAL TV, INC., charges indicated in the following month's SOA (effective the next month's billing). Processing will be within 5 days before SOA due date.
